Forms

We have set up a FERPA-compliant system for you to submit electronic forms, rather than emailing them.  Please submit all electronic forms and any applicable supporting information by following the instructions below.

University/College forms:

The CALS Student Forms webpage can be found at https://www.cals.vt.edu/academic-programs/current/CALS_Studentforms.html.  For any Online Forms, students will meet with their academic advisor to discuss changes in their academic plan, then fill out and submit the appropriate form on the CALS Student Forms webpage.  Students will receive confirmation that the submission has been received, and confirmation when the process has been completed. Students will also receive an email if further information is needed.  For PDF forms, complete the following steps for submission:

  1. Download the PDF to your computer.  Complete the form using Adobe Acrobat Reader (free download; don’t download any other offers).
  2. If a form requires your signature, please sign it with the Adobe Acrobat Reader signing tool.
  3. Save completed form again before uploading into the tinyurl.
  4. Submit completed form here: https://tinyurl.com/AnSciForms
  5. We will secure any additional signatures and forward to the CALS Academic Dean’s office.

Major, Minor and Option Changes

To add a second major, add or drop a minor, or add or change your option, please submit your request directly to CALS via the CALS Student Forms webpage: https://www.cals.vt.edu/academic-programs/current/CALS_Studentforms.html

 

UR/IS (Undergrad Research/Independent Study) forms:

Independent Study: An Independent Study is typically a continuation in greater depth of a topic covered in a regular course, and allows students to study topics of particular individual interest. Specialized instruction related to design teams are most often independent studies. 

Undergraduate Research: Students can earn Undergraduate Research credit for research projects they carry out under faculty supervision. The research typically creates an artifact such as a paper, presentation, or technical product.

Graduation Analysis
The APSC major offers Graduation Analysis as a recommended service for students to evaluate academic plans and establish objectives for on-time graduation.  If there are any concerns regarding your academic progress towards degree completion, you and/or your advisor are strongly encouraged to request a graduation analysis.  Please complete the form below to apply for Graduation Analysis.

Once you have applied for Graduation Analysis, our advisors will evaluate your course plan, and one of us will contact you to schedule your Graduation Analysis appointment.
